Registration Fee:   $400 (until October 12, 2009)
Late Registration Fee:   $450 (after October 12, 2009)
Guest Fee:   $100


Registration Transfer and Cancellation Policy: Registrations may be transferred to another individual at any time and without penalty. Cancellation requests received no later than October 12th will be refunded at 100%. Cancellation requests received October 13th through October 18th will be refunded at 50%. Cancellation requests received after October 18th will not be refunded.

"Thursday Only" Registration Fee includes Thursday's Breakfast, Lunch and Breaks, and full participation in Thursday's Business meeting. Attendance at Thursday evening's Dinner and Dance may be added for an additional $75.

"Friday Only" Registration Fee includes Friday's Breakfast, Morning Break, and full participation in Friday's Business meeting. If you are arriving early, Thursday evening's Dinner and Dance may be added for an additional $75.

PLEASE NOTE: The amount calculated by the website will reflect the normal 2-day registration fee schedule, so it's important that you indicate that you are registering as either a "Thursday Only" or "Friday Only" attendee during the registration process. You can do so by indicating the type of registration in the "Special Considerations for Registrant and Guests" box that is available during the registration process. Golf Fee: $0 per attendee/$15 per guest

Agenda Questions for the Meeting Discussion
The success of the meeting depends upon your questions and subsequent discussion. You'll receive response and insight from some of the most knowledgeable individuals in the insurance industry. Questions should be submitted through the LHCA website. Submit your questions by October 9th & your name will be entered into a drawing. One entry per question! Questions submitted after the 12th will be supplemental.

A compilation of all questions submitted will be e-mailed to registered attendees approximately two weeks prior to the meeting date to allow attendees the opportunity to review and prepare for discussion at the meeting. Attendees will need to print the questions and bring them to the meeting.
